We are seeking a Licensed Master Social Worker (LMSW) to provide remote telehealth services for a multi-specialty practice serving patients in the Bronx, NY area . Qualifications:

• Education: Master’s degree in Social Work (MSW) from an accredited program.
• Licensure: Active NY State LMSW license 
• Experience: Open to new graduates
• Language: Must be fluent in Spanish
• Technical Skills: Familiarity with social work best practices, case management, and patient advocacy.
• Soft Skills: Strong communication, empathy, problem-solving, and teamwork abilities.

Key Responsibilities:

• Provide psychosocial assessments and therapeutic interventions.
• Collaborate with healthcare teams to support patient care plans.
• Advocate for patients and connect them with community resources.
• Maintain accurate documentation in compliance with regulatory standards.
• Participate in case conferences and continuing education opportunities.
