Position: Office Triage LPN

Company: Twin Cities Physicians

Location: Golden Valley, MN

Schedule: Full Time | Monday–Friday

Pay: $24–$27 per hour

As an Office Triage LPN at Twin Cities Physicians, you'll work in a fast-paced office setting where your clinical skills and communication will directly impact patients, families, and providers across senior living communities. This is not your typical desk job— every day brings new tasks, real-time clinical coordination, and opportunities to make a difference .

Your Day-to-Day Responsibilities:

• Triage incoming phone calls from patients, families, and facility staff
  
• Collaborate with providers and admin teams to ensure timely patient care
  
• Process and review provider orders, standing orders, and clinical updates
  
• Reconcile medications, prepare charts, and update EMR systems
  
• Respond to faxes, calls, and internal communications with professionalism and urgency
  

You’re a Great Fit If You:

• Hold an active LPN license in Minnesota
  
• Have experience in senior care or primary care settings
  
• Are confident using EMR platforms like PCC or Eldermark
  
• Thrive in fast-paced, multi-tasking environments
  
• Pay strong attention to detail and value teamwork
