We are seeking a dedicated Locums Internal Medicine, Family Medicine or Geriatrician Physician to provide high-quality outpatient care across multiple Long-Term Care (LTC) and Assisted Living (ALF) facilities in Hickory, NC, Roanoke Rapids, NC and Columbia, SC areas. This role focuses on delivering compassionate, patient-centered care to an aging population while supporting facility staff, patients, and families throughout the post-acute continuum.
Position Details
Setting: Care in multiple Long-Term Care and Assisted Living facilities
Location: Physicians will cover multiple locations
Full and part time options available
Schedule: Monday–Friday, 8:00 AM – 5:00 PM
Daily Patient Volume: 18+ patient visits per day
Responsibilities
Provide comprehensive care to residents in LTC and ALF settings.
Manage chronic medical conditions and acute issues for geriatric and medically complex patients.
Conduct daily patient visits, ensuring at least 18 encounters per day.
Collaborate with nursing staff, therapists, and facility administrators to ensure coordinated care.
Offer clinical expertise, communication, and support to patients and their families.
Serve as an advocate and resource as patients transition through:
Acute Inpatient Rehab
Skilled Nursing
Long-Term Care
Assisted Living
Transition back to home
Maintain thorough documentation and follow facility protocols.
Requirements
Board Certified or Board Eligible in either:
Internal Medicine
Family Medicine
Geriatrics
Active North Carolina or South Carolina medical license
Experience in post-acute, geriatric, or facility-based care preferred
Strong communication skills and ability to work well in interdisciplinary environments
Compassionate approach to complex and aging populations
